TODMORDEN GROUP PRACTICE RUNS A TRAVEL CLINIC ON FRIDAY AFTERNOONS BETWEEN 4pm and  6pm, WITH ALL CLIENTS BEING ASKED TO COMPLETE A TRAVEL HEALTH RISK ASSESSMENT FORM PRIOR TO ATTENDING THE CLINIC.

THE NURSING TEAM WILL THEN CONTACT CLIENTS TO ARRANGE THE REQUIRED APPOINTMENTS, WITHIN 7 DAYS OF RECEIVING THE FORM.

FORMS AVAILABLE AT RECEPTION BETWEEN 8am and 6.15pm.
